      About 3 months ago, I started experiencing heart palpitations at night. A few times a minute, my heart would have a single strange beat, almost like a twitch in the heart muscle. This got worse over about 3 weeks, with twitching every so often. After 2 weeks, I went to the ER to make sure I wasn't going to die. They did blood tests and an ECG (during the ECG, the palpitations weren’t happening), and they said everything seemed fine. After the visit, the palpitations decreased over about 2-3 weeks, but the twitching started to increase, reaching a point where it was almost constant for a few days—mostly in my eyes and face, but also in my legs, hands, and stomach. Now, the twitching doesn’t happen every day, and the heart palpitations only happen a few times a day.

      When it started, I was weaning myself off synthetic T3/T4, and by the time I went to the ER, I had been off thyroid medication for ~2 weeks. Now I've started NDT (8 days ago), but it hasn’t made any noticeable difference. So far, I’ve tried pregnenolone, progesterone, magnesium glycinate, taurine, aspirin, vitamins E, A, K, and D, and a B complex—none of these have helped to any degree. Almost every day, I drink 1/2 gallon of 1%/skim milk and 3 cups of orange juice, and I occasionally eat beef, so I don't know of any deficiencies but my diet isn’t completely free of toxins/additives. Here are the results of the ER blood test:

          @MooseZoomer I had the same situation, have it but mildly now. got to a point where i was pretty sure i was close to death also. had extreme twitching rippling all over my body, heart pounding & beating erratically in crazy rhythm day to day, involuntary jolting & jolting out of sleep with adrenaline rushes etc. at its peak i wrote a note in case i died in my sleep

          conclusion i got to is on a basic level its a major lack of atp production, fits with the timing of you stopping the medication
          Ray wrote when the ATP production in the cell is high the cells can stay relaxed

          When the cell is energized, by the mitochondria working with thyroid, oxygen, and sugar, these proteins rapidly change their form, binding calcium and removing it from the contractile system, allowing the cell to relax, to be fully prepared for the next contraction. If the calcium isn't fully and quickly bound, the cell retains extra water and sodium, and isn't able to fully relax.

          for me it was Vitamin D + magnesium that took it to an extreme. in some situations people convert vitamin D excessively to the 1,25 form and it potently stops mitochondria producing good amounts of ATP. In hypothyroidism people have higher 1,25 vitamin d. and in chronic inflammation through macrophages

          i tried magnesium and pushed through with different forms because its commonly recommended for this, nope it accelerated things way worse. magnesium is used for enzymes that convert vit D forms.

          when i stopped them both for a month or so the problem eased to about 10% ,
          i noticed the twitching was worse in caloric deficit too (lower atp, and more vit D released from fat cells through lipolysis)
          your low creatinine points to malnutrition, low muscle mass or or a failure to break down muscle for energy in low calories or low protein diet, how many calories do you get?

          while u have this problem i would cut that milk intake / calcium in half at least if not more, likely way too much

            This is my opinion
            Not medical advice….
            I would

            1- stop fucking around with thyroid medication.
            2- make sure you are EATING enough
            Because…
            3- you are drinking so much liquid- you could simply need to significantly up your SALT intake

            So-
            Get off all your supplements
            Let your body find balance
            Eat robustly
            Up your salt

              You're suffering from low inflammation grade.
              Taking a supplement T3 and T4 when you metabolize badly / insufficiently won't solve the problem on a long term.
              If you've got too much estrogen (fatty belly) or high cortisol (stress), some endocrine disruptors, a lack of useful nutrients for your thyroid, you won't recover.
              Not exhaustive.

                    Some things to look at:
                    Elevated white count -> Get an ultrasound to check for Pericarditis and other hidden infections (gum disease, wisdom tooth sockets or other cavitations - get a cavistat scan). Check appendix, etc).

                    Drop any extra supplements and aspirin, you may be over thinning your blood. Other advice here is good, stop messing with thyroid meds unless a need is biochemically confirmed.

                    Start meditating and learn how to relax your breathing into your belly.
