        @jakethesnake read up on ray peat estrogen suppression

        I'm also using exemestane which is very good at nuking estrogen but thats because I'm also taking stuff that radically increases estrogen

        So you probably don't need exemestane

              @alfredoolivas I have yeah, but what was your dosage like? I’m doing like 3 ounces three times a week right now.

                  Have you tried red light therapy? 2 x 15 minutes day? When I had acne (more than a decade of my life) it had a prevention effect. But I didn't see results in the first 3-4 weeks, so some patience is required.

                  Is there scar formation? Listen, I feel for you as a former acne patient who experienced a lot of emotional pain that somewhat still lingers today. So I get why you want to try accutane as a last resort. But that stuff can be dangerous with potentially life-long health effects (liver health etc.) Even if you develop scars, that's still better than a damaged liver going forward.

                  How is your digestion? Although I'm not a fan of carnivore at all, I'm a bit surprised you didn't see any effect on your acne when you tried that diet. For how long did you try?

                  Have you tried oysters (zinc)?

                  What do you use for skin care? I know it doesn't solve the causes for acne, but could give you some relief/improvement. A serum with azelaic acid or retinol or niacinamide could be helpful.

                  You could also try a Lactoferrin supplement. It's a pretty safe supplement.

                  I've never been a big party attender, but I never went to a party where I didn't probably offend most of the people there by talking about what I was interested in. (Ray Peat)

                    @jakethesnake I'm a little older than you, apart from keeping healthy thyroid/metabolism through your preferred means, I've found that sunlight and or swimming in the ocean is the only thing that can consistently clear my skin. For me, this is far superior to paying close attention to my diet and any sort of supplementation or topicals - I just get way better results for such little effort.

                    For the record, I think if you're otherwise healthy, taking Accutane at <20mg/day can help curb the purging and maybe even mitigate the hepatotoxicity. A bit of a crutch if you've already "tried everything under the sun" and want something to try that is a little hands-off. You'd likely want to add TUDCA and or NAC to your stack to help though, I supposed BPC wouldn't hurt either?

                    I've tried liver before for health reasons and it did help with my skin - even when consuming only 1x per week. Though it tasted a bit unpleasant and could not find a reliable source for store bought liver.

                      @hawk1 thanks for the help man, yeah, I live in Southern California, so I get plenty of sunlight and I do go in the ocean often, I think I may just go ahead with the low-dose, what is the BPC you’re referring to thanks.

                        @Luke hey man, thanks for the ideas, I don’t specifically use a red light machine, but I get plenty of sunlight a.m. and p.m. for topical I use azalaic and niacinamide. And I just found them to dry my skin out and hurt my barrier. Yes I do eat oysters once a week. Thanks for the help.

                          Judging by the thread so far, the last thing I'd suggest before trying Accutane is maybe some antibiotic if you haven't already - a lot of stuff on here about that.
