Recently Jordan Peteron's daughter made a video taking about his bad health, with akathisia being his main problem.
SSRI-induced akathisia can result from excessive serotonergic activity, through activation of the 5-HT2A receptor. Increased 5-HT2A signaling is thought to suppress dopaminergic transmission in the striatum, leading to a relative hypodopaminergic state.
This reduction in dopamine activity then triggers compensatory overactivation of both the adrenergic and cholinergic systems, contributing to the characteristic symptoms of akathisia, including inner restlessness, agitation, and an urge to move.As described by Anton J.M. Loonen and Stephen M. Stahl in The Mechanism of Drug-induced Akathisia:
“Much evidence exists to support the idea that akathisia is related to a decrease in dopaminergic neurotransmission. This likely results in decreased activity of the entire ventral striatum. It can be postulated that this decline results in compensatory enhancement of the activity of adrenergic projections from the locus coeruleus complex. Because these projections selectively stimulate the shell portion of the nucleus accumbens, a mismatch between the activities of the NAcbC and NAcbS results. Relative overstimulation of the NAcbS results in the typical urge to display senseless ‘curious’ or ‘defensive’ behavior, and is accompanied by dysphoric feelings.”
Agents with strong postsynaptic 5-HT2A antagonism have shown potential benefit in the treatment of akathisia, including Ritanserin, Cyproheptadine, Trazodone, Mianserin, and Mirtazapine.
“Accumulating evidence during the last two decades indicates that agents with marked postsynaptic serotonin 5-HT2A receptor antagonism (ritanserin, cyproheptadine, trazodone, mianserin, mirtazapine) may represent a new class of potential anti-akathisia remedies.”

In addition, it is generally considered to have a relatively favorable side-effect profile compared to many alternative treatments.
There is a very recent controlled clinical trial which should that cyproheptadine was effective as a treatment for akathisia.
"Cyproheptadine significantly reduces symptoms of antipsychotic-induced akathisia. Based on the evidence of this study, particularly the ARR and the NNT for achieving complete remission, it seems that cyproheptadine could be a potential alternative for managing akathisia induced by antipsychotic medications, especially in comparison to previous studies involving mirtazapine and propranolol."
