This seems to be the consensus protocol from the forums:
- Start with T3 monotherapy, at a few micrograms, one to three times daily
- Closely monitor symptoms and vitals
- Slowly increase T3 dosage and frequency over weeks; daily adjustments should be avoided
- If/when sleep and cardiac issues show up, which they likely will, add in T4 in the evening
- Every couple weeks adjust dosage and frequency of T3/T4 until full symptom improvement
Best dosage and frequency (based, again, on my reading of forums and anecdata) is ~5mcgs T3 every 2-3 hours, with 20-60 mcgs T4 in the evening, in one or several dosages, with T3.

To the small minority of users who make homemade kefir, what grains do you use? I’ve responded positively, albeit modestly, to store-brought kefir, and want to increase the potency by fermenting my kefir at home.

@evan-hinkle Yes, I’ve heard that too.
I think once T3 levels are brought back in range, either with endogenous production or exogenous supplementation, OP’s pituitary will stop producing as much TSH. Right now it should be regarded as a sign of general thyroid dysfunction, as the TSH negative feedback loop is not being activated.

Consider that your thyroid is producing adequate hormones, but extrathyroidal conversion of T4 to T3 is impaired, as evidenced by the sufficient T4 and low T3. One therapeutic route would be detoxifying your renal and hepatic conversation pathways; for this, selenium and iodine would prove ineffective, while direct supplementation with synthetic hormones, which would place your body in a euthyroid, pro-metabolic state, may promote natural detoxification.
